Answering “fades quickly, spread across multiple teeth, stable, temperature-triggered, nothing visible” points toward ordinary sensitivity. Answering “lingers, one specific tooth, getting worse, worse with biting, something visible or textured” points toward a cavity worth having evaluated soon.

A crack that’s caught early, while it’s still limited to the enamel and outer dentin, is often manageable with a crown or bonding to stabilize the tooth. Left untreated, the same crack can progress deeper toward the pulp, at which point a root canal becomes necessary — and in the most advanced cases, the tooth can split in a way that isn’t salvageable at all. The earlier it’s addressed, the more of the natural tooth can typically be preserved.
When “just sensitivity” is actually a cavity in disguise
A few patterns are worth treating as a signal to stop guessing and get evaluated:
- Pain that’s gotten noticeably worse over the past several weeks rather than staying the same
- Discomfort that’s isolated to one specific tooth rather than spread across several
- Pain that lingers for a minute or more after the trigger is removed
- Sharp pain specifically when biting down on that tooth
- Any visible dark spot, rough patch, or hole you can see or feel with your tongue
